To help ensure a smooth start to your care, please submit your insurance information as soon as possible when scheduling a new appointment. While I am happy to support you in navigating insurance questions, it is ultimately the client’s responsibility to verify benefits, including coverage for out-of-network services, co-pays, and session limits.
Superbills for Insurance Reimbursement
For clients with out-of-network insurance, I provide superbills that can be submitted to your insurance company for potential reimbursement. While I do not bill out-of-network insurance directly, superbills make it easier to access out-of-network benefits.
Sliding Scale Options
I offer a limited number of sliding scale spots to assist individuals who are facing financial hardship in accessing care. Please contact me to discuss availability and eligibility.
Fees (Out-of-Pocket Costs for Self-Pay Clients)
● Intake & Biopsychosocial Assessment /SUD Assessment and Mental Health Assessment for Gender-Affirming Care (55 Minutes): $200
● Individual Psychotherapy Session (53 Minutes): $150
● Couples Psychotherapy Session (53 Minutes): $180
● Documentation Requests: $50 (includes ADA, FMLA, and treatment letters)
● SUD Assessment Documentation for Legal/Court Purposes: $200 (includes coordination with the appropriate individual associated with your case)
● 15-Minute Phone Consultation: $0
These fees represent the out-of-pocket cost for self-pay clients. Please contact me if you have questions about billing, insurance reimbursement, sliding scale options, or specific documentation requests.
Cancellation Policy
I understand that unexpected events can come up, and your time is valuable. If you need to cancel or reschedule an appointment, I kindly ask for at least 24 hours’ notice. Cancellations made with less than 24 hours’ notice and missed appointments (no-showing after 10 minutes) are subject to a $150 fee, which is not covered by insurance.
